David A. Jobes, Ph.D., ABPP
David A. Jobes, Ph.D., ABPP, is senior partner at the Washington Psychological Center. His clinical practice includes assessment and psychotherapy with teenagers and adults as well as clinical consultation and forensic work. Dr. Jobes is a Professor of Psychology and Associate Director of Clinical Training at The Catholic University of America. He is also an Adjunct Professor of Psychiatry, School of Medicine, Uniformed Services University of the Health Sciences. His primary area of research is in the area of suicide prevention and clinical suicidology; he also has expertise in professional ethics and clinical risk management. Dr. Jobes is a Fellow of the American Psychological Association and is board certified in clinical psychology (American Board of Professional Psychology).

Below are copies of forms that Dr. Jobes uses in his practice. The first is a brief intake form, which allows Dr. Jobes to gather some basic administrative and clinical information about you. If you have time then it is helpful, but certainly not mandatory, to fill this out before your first session. Copies will also be available in the waiting room. The second form is an Agreement for Services that spells out some of the expectations of the professional relationship, as well as the legal limits to confidentiality. You do not need to download this and print it out. Dr. Jobes will have a copy ready for you. This electronic version is provided for your reference. You will also be provided with an addendum to your agreement during one of your initial sessions that will detail Dr. Jobes' specific business policies.
