Steven A. Wong, Ph.D.
Dr. Steven Wong earned his Ph.D. in clinical psychology from The Catholic University of America in Washington, DC in 2003. He is licensed in the District of Columbia and is a member of the American Psychological Association. His clinical practice involves providing individual therapy for adults with a wide variety of presenting concerns, including relationship problems, mood disorders (e.g., depression and anxiety), addictions, trauma (e.g., sexual abuse, combat trauma, etc.), and more. He has additional subspecialties working with men and college students in psychotherapy.

Below are copies of forms that Dr. Wong uses in his practice. The first is a brief intake form, which allows Dr. Wong to gather some basic administrative and clinical information about you. If you have time then it is helpful, but certainly not mandatory, to fill this out before your first session. Copies will also be available in the waiting room. The second form is an Agreement for Services that spells out some of the expectations of the professional relationship, as well as the legal limits to confidentiality. You do not need to download this and print it out. Dr. Wong will have a copy ready for you. This electronic version is provided for your reference. You will also be provided with an addendum to your agreement during one of your initial sessions that will detail Dr. Wong's specific business policies.
